Did you wash your hands? Did the person before you? Mysophobia anyone?

I cannot tell you how agitated I get when I witness some habits of people and their ability to affect (infect) my life.

 

No, I am not a compulsive, mysophobic, although there are times I wonder what is so wrong with practicing a little mysophobia anyway.  Maybe those with the disorder are on to something.

 

Ever ride a bus or subway, or any vehicle used for mass transit?  Ever sit and watch the people, sneeze into their hands, eat, cough, and pick at things and then grab the handrail or door handle and you say to yourself “gee, what a pig?”  Ever not see it and think you are safe and never really think about the people on that same ride BEFORE you.  Notice how many supermarkets have the sanitizing wipes by the carts now?  I read somewhere that 78 percent of shopping carts have fecal matter detected on them.  What you can’t see sometimes can infect you.

 

Today, I had the displeasure of having one of my all time pet peeves witnessed.  I was in a public restroom at a McDonalds.  It was clean enough.  After doing my business, I washed my hands.  I lathered, I scrubbed, I cleaned.  Then I went looking for a hand towel.  None.  They had these blowers to dry my hands.  Okay.  I can push on it with my elbow.  I can stand there for another minute or two, and viola, clean dry hands. 

Just then, it happened.  Two young girls and their mother came out of the other stall.  Mom with a baby in one hand and a diaper in the other.  They opened the bathroom door and left.  Each grabbing the handle.

 

Now even though my hands are clean and dry, I don’t know what I am touching there – I feel trapped in the bathroom. 

 

I remember when the paper towel was there.  We were taught to grab a towel, turn on the water, wash, turn off the water using the towel, and using the same towel, open the door and toss the towel on the way out. 

 

Now, no matter how much we practice safe and clean habits, we have to wonder about the habits of others. All in the name of saving the neatness of the bathroom?  I would much rather walk into a bathroom with hand towels on the floor next to the exit, than to wonder who touched the door before me on the way out. 

 

Maybe the sink should be on the outside of the bathrooms.

Random thought of the day: Some words are overused. Here are some of my pet peeves.

I have watched and listened to many people over the last few days that has led me to form the opinion that certain words are overused, and used with no sincerity or knowledge of meaning behind them.

Innocent:  People use this word to describe others or their own actions.  If an action is questionable, which is the only reason someone would have to use ‘innocent’ to describe it, it is hardly innocent.  It may be harmless, asinine, thoughtless, or sometimes even more sinister – but hardly innocent.

Juvenile:  I am sorry but when someone over the age of 18 does something incredibly stupid, it is just that – incredibly stupid.  Many juveniles don’t act that way until they see adults being stupid.  let’s stop using a term to describe adolescents to describe adults behaving badly.

Handicapped:  Just because an individual has alternative ways of capably accomplishing something does not make them handicapped.   However, there are some who may be considered juvenile (see above) that are just incredibly stupid, which in my mind, makes them more handicapped than some who must use alternative ways to achieve a means to and end.

Difficult:  All things are difficult until you learn them.  We learned to sit, eat, walk and talk – we can learn just about anything.  It is the wanting to learn and do the best we can that is difficult.  Everything else is just work.

Income: I keep hearing this word, but have not seen an ‘income’ without the ‘outgo’ that goes with it, starting with a large percentage of my ‘income’ outgoing to the IRS.  We should probably call it, residuals.

Insurance:  Now here is a word that throws me.  It should really be called ‘assurance’, because if you don’t have it you are assured to feel screwed (especially with the new laws) when you need it, and if you have it, you can be assured that you will be more stressed than you were before you needed it for health, home, or vehicle reasons.  I believe big Pharma loves this mechanism that happens to keep our blood pressure raised and minds confused because it allows them more government funding for research.  Funding of course that comes out of our ‘residuals’ (see above).

Respect:  Some people say they deserve this, but have never earned it.  This is, in my mind, one right you are not born with.  Sometimes you can earn it quickly, but most times it takes work and if you have it, you should never get comfortable with it, because if you act juvenile (see above) you can lose it faster than you got it.  I also believe you must have it yourself ,for yourself before someone else can share theirs with you.

Employment:  I can’t tell you how many times I personally have heard people say they want a job, but then don’t ‘show up’ to do the job.  By showing up, I do mean physically and mentally showing up.  I have also personally heard people, even in this economy, say things like ‘that’s not my job’.  I believe, that if you want to work and you receive a paycheck for legitimate tasks, and you happen not to appreciate the tasks assigned to you, you have three basic choices.  Everyone does.  One – do it with a smile.  Two – do it and ask for a raise.  Three – move on and allow someone who appreciates the opportunity to earn a paycheck to earn it.  Sitting at your job, not doing your job, hurts everyone.  Not just you and your employer, not their clients, but also the person who could be there, earning a wage and taking responsibility for their lives.  So it is not employment, but work.  Employment is using your abilities to get the task done in your job, which you do get a paycheck to do.

Love:  This word most of all is over used.  People, especially the young, use the phrase ‘I Love You’ so easily and freely, they lose the whole idea behind the words.

I believe that when you love someone those words should come from the heart and from the knowing.    It comes from wanting to be with that person, caring for that person, being there in times of need for that person, and them there for you, yet being able to be apart without a suspicious thought rolling around your head when you are.  Loving someone for who they are, beyond that ‘honeymoon’ stage that young couples have.

An extension of  overuse of ‘I love you” is  ‘I want to share my life with you‘.  When in that honeymoon stage of ‘love”, people do not share lives, they share time.  They forget, or do not know, or do not realize that sharing a life together means sharing the work, the play, the bills, the responsibilities, the friends, and even the time alone, apart from each other.  When you can do that and still lose your breath when thinking about the other person, that is love.

 

I do believe that when you love someone, truly love someone, those words should come freely.  It should never be something that you can hold back.  It should just overwhelm you to where the words come out, at the most peculiar times.  I believe that if you love someone, it is the first thing you want to say in the morning and the last thing you say at night – just because, if you lose the next moment to say it, you are fully aware that they knew exactly how you felt.  But it should not be something that is given to every person you know so easily.  It takes away from the meaning.

 

These are just a few of my pet peeves of overused / misused words.  Do you have any?  Who knows, maybe others share them with you.  I would love to hear them.

Proud to be an American Business! Happy Independence Day!

Growing up, the fact that I was an Army brat was not as significant as it has become in my adult years.  My childhood wasn’t regimented as some see in the movies, but respecting the fact that my dad was a military man came easier than listening to the rules of the household.

 

I remember the “Made in the USA” commercials urging people to buy American.  I miss those ads and feel we need them to be resurrected.

Nowadays, finding anything made in the USA is difficult as more and more work is being sent off shore, and more and more imports are accepted and purchased here in this great nation.

This continues even after recent horrible events where products made overseas have been found to be poor quality and even causing death to humans and animals.

Now I am not saying this would not occur here, but we will never know because the cheap (not inexpensive) products are still utilized here because, well, the fact that we off shore so much HAS affected our economy.  Couple that with the cheap and dangerous labor practices that other countries have, and who are we really helping here?  So many companies and celebrities complain of sweatshops and inhumane practices, but they still find a way to focus energy on using these same services because it saves them a buck and costs the American people jobs, affecting the US economy.

 

I have said many times, even our personal security is at risk due to the offshoring of our medical and non-medical transcription projects and work.  Even your insurance claims for your health, worker’s comp, car and home owner’s insurance  are now sent off shore to be processed.  Think about the information being shared with those who do not have to follow our rules and regulations.  And you worry about healthcare?  So now everyone will be able to be exposed to this risk.

 

Journalists who use these offshore transcription mills run the risk of their hard work being “taken” by someone else and there is nothing that can be done about it.  A company’s recordings of roundtable discussions, meetings, etc, all sent to offshore mills with that same risk.

 

So, as the Fourth of July approaches, and I hear news of some of more and more of our troops coming home, I celebrate the fact that all of CLK Transcription’s work is done here in the USA.  CLK Transcription, Inc. is proud to be an American Business.

In my opinion, here is what makes a good businessperson…

In the past several weeks I have been met with some professionals (and I use that term because they use it only) that I was giving my business to.  I was their client, their customer, their money in the drawer during this sucky economy.

 

The first was a professional who did construction, maintenance – you know the ‘all around handy guy.’  When asked to do emergency work that was being paid for by me, it ended up being a four-day job with no work and no communication.  I moved on.

 

Then it was the mechanic who floored me in a number of ways.  First I asked if the job could be done in two days, because well, I needed my vehicle back in two days, and I told him politely that if he couldn’t, I would have to find someone else to do the job, if possible, in two days.  (just a tire rod replacement and inspection, which I know only takes a couple of hours).

He was offended and told me I should show him more respect. What?  Am I not paying him?

Oh and the second way he floored me.  He called me to tell me it was done, and then said I am going to lunch at 12 so be here before then or wait for it.  Now I was on my way, about 20 minutes out, and it was 11:30.  You would think rather than putting me out, and making me wait to PAY him, he would hang around for a few minutes IF I were late – he does own the shop after all.

After paying, I was then harangued about how I should show respect for the good work that is done and not threaten to take the work elsewhere.  Well, I am a customer.  I pay for what I want.  If you can’t give it to me, then I will find someone who can.  That is the price you pay for calling yourself a professional.

I should respect him even though he can talk to me, a paying customer that way?  I think not.

 

Here’s how I treat my clients.  When they need something I either deliver it, or they go elsewhere.  I certainly wouldn’t tell them take it or leave it.  I certainly wouldn’t go to lunch and not accept a payment, making them wait to pay.  i wouldn’t put their work on the back burner because they had a rush job.  I work harder to show them I can do it, and do it the way they want.

 

Has this become a primitive way of thinking?

 

 

In this economy, I think the best way to maintain clients – not just get them and move on – is to treat them like paying clients.  because well, they are.  If I can’t give them what they want, why waste their time.  By being honest, they may just come back next time to me when I can give them exactly what they want.

They may just spread the word about how wonderful their interaction is.  They may even refer others tome. And I have found they do.

 

Now, I firmly believe that when you get crappy service, you should mention it.  And many will repeat the information about the crappy service over and over and over again.

But let’s not forget the good service.  As a client myself, when I like something, I share and share often.  I refer and promote and recommend all the time.  That is the respect they EARNED when I do business as a paying customer.  Notice, I haven’t shared the name of the crappy customer service companies.  I won’t give them weight!

 

That is the respect I try to earn as a business owner/operator for my clients.

 

So, if it is primitive thinking that gets me here, then so be it.  It works for me.

With all that you have to get done, it’s okay to ask for help.

As a journalist/writer, when you have a project that has been assigned, there are so many steps to get through to be able to develop the perfect piece, the perfect end-product, that it’s a wonder you have enough time in the day to even think about them all.

Need a US medical transcription company? CLK Transcription does that too!

Although I have a deep and profound love for the journalist, ghost writer, and others who need non-medical transcription, my roots are in the medical field beginning many years ago with insurance verification, medical records, home health care administrator, and then on to medical transcription with another company, I do enjoy the medical client as well.

I treat each and every one of my clients as my only client.  Each one has their preferences, demands, and expectations.  I could never cookie-cutter their work.  Especially the medical reports.

I know that when a client needs me, they need me, and I give them everything I can.  I work with their deadlines, their requests, and their sometime outrageous expectations with a smile and always try to meet even their hardest challenge.  I haven’t had problems yet, because I have a great team behind me and we love the challenge.

Medical transcription is even more difficult – with accuracy and format being most important for strict legal and continuing health care concerns.  I worked with JCAHO and state agencies during audit periods and know that reports should always follow format throughout the practice of a physician.  When there is a difference, the agencies auditing sometimes feel those reports could have been “amended” to pass an audit, rather than original reports. Many other transcription companies think they know best and make changes because it works for them, without consideration for the client’s demands

The TAT for medical reports is extremely tight and must be met.  There are times that a patient’s care is on hold for the return of reports. And if there is any question of the information within the report, this too can hold up patient care. There is no guessing with medical reports.  The difference between b.i.d. and t.i.d. is costly and could mean a life.

A physician is under extreme pressure to ensure that all information is documented accurately and timely.  Many hospitals even require physicians to dictate every change in medication and other items that used to be documented in the chart, but not in report format.  This requirement is to lower the risk for malpractice claims. I think it is wonderful because it means better continuity of care for me – the patient.  It comes with a price though.

These demands placed on physicians has been a cause for increase in fees.  The physicians have had to hire in-house staff to handle the transcription, and in those cases, must offer all the benefits that employment calls for.  If they choose to send their work outside of the office, many have chosen to send it offshore to save money.  This is costly to the patient and the doctor.  Costly o the patient because their information – everything from name and address to family history, social history, and all medical information is now available in a country that does not have to answer to our regulations.  Costly to the physician because when you hire someone who has English as a second language, you will ultimately get a report with the same second-rate English causing second-rate information in reports that must be proofed and edited by another staff member – or the physician himself – which costs even more time and money.

Because I strive to keep the US transcription of all of our US-based clients within our borders I have kept my rates lower, hopefully to save the client, but also to allow the work to be transcribed within the US, safely and at the same time by US transcriptionists who have paid thousands to educate themselves in this field and thousands more to maintain their training and education – and understand the English language .

So, yes, I do profoundly enjoy the non-medical portion of my business, but the medical side is also my heart and a large part of the enjoyment of what I do.

Dear interviewer, you are a great interviewer but…a Transcriptionist’s point of view.

One of CLK Transcriptions independent contractors asked why we are never interviewed for a story.  Then she realized we are too busy transcribing your audio to stop.  She did however have some thoughts and suggestions on how to make transcribing your audio of other interviews a bit more transcription friendly – whether we transcribe your audio, someone else transcribes your audio – or even YOU transcribe your own audio.

 

Many of these I have mentioned before, but I had to share her comments as a worker bee trying to save you time and money.

 

 

Dear Interviewer:

 

Please check your equipment.  Growls, howls, static, etc, in your audio make accurate transcription difficult, but knowing ahead of time helps me to prepare.

Also, when interviewing outside, in a crowd, or say with an ambulance passing by during the discussion, understand that unless you repeat some of the conversation when that pesky ambulance drove by, there may be some blanks.  Try to consider better placement of your microphone in those situations.  The best place for the microphone is not next to the plate and glass, or even the paperwork that you will be moving around.  We hear the noise first.  It isn’t that we can’t do it; we love the challenge, but proper placement leads to better end product.  We understand it isn’t always possible, so again, just give us a heads up that you are aware the audio is difficult and that helps us prepare as well.  Also, you may want to be sure that you have the interview and not just your side of the conversation on the audio.  Listening to the audio yourself also helps when you think you are sending a recording thinking it is one thing and well, it isn’t.

 

Those words you spoke when yawning or belching, or coughing and sneezing, really are hard to decipher.  Could you repeat that please?  It is impolite to stop in the middle and speak to someone else – we do not know if you want that transcribed, unless you are ordering dinner.  And please, as busy as we know you are, please leave the chewing and bathroom breaks out of the audio.

Please allow your interviewees to finish a sentence without interruption whenever possible.  I can’t decipher his/her responses accurately if I can’t hear them fully – and we hate having a blank in your report.  And I always wonder whose part of the conversation is the most important and should be in the transcript when I can’t clearly hear both speakers and you ask me to clean it up for you.

Please call your interviewees by name early in the conversation so his/her name can be inserted during the article rather than “interviewee”.  A find-replace only takes a few seconds but keep in mind I’m paid by the audio minute and seconds matter.

When an interviewee is using a “unique” technical or medical term, please repeat the word – or send it with the audio in your e-mail.  Hearing it correctly will help me search for the correct spelling.  This would be especially helpful when you are interviewing someone whose accent is other than American English.  When possible, especially with those pesky obscure names, spelling it helps.  Spelling HABITAT doesn’t; we know how to spell that, but when you have a name like Nahuat, spelling helps.

Also, when possible, giving us a bit of a background – speakers names, websites, etc – on the topic of the discussion.  This helps us research words and phrases so you don’t have to.   We do not all watch every TV show produced, so even the title of the show so we can research names and characters, is a help.

Also verifying the length of the audio you are sending so we know there was no problem with the download.

Let us know if you need the file immediately.  It may mean a stat fee, but this is not always the case.  It helps us help you when we know your projected TAT for the audio.  Whether its needed later int he day or the next morning, even if a week away.  (Yes, we have clients who actually give us a week to turn around a document, but we haven’t ever taken advantage of that yet.)

Recording and transcribing chitchat is a waste of your money.  I understand the importance of the “break the ice” conversation at the beginning of an interview.  If this conversation is not necessary to the article you are writing, please let us know when to start transcribing and we will skip it and we both save time and money.

Take into consideration that to transcribe your audio is at minimum three times the length of the audio – total transcription time and proofing.  And that is with a clear audio.  If you have a stat/priority that is four hours long, don’t expect it by evening if sent at 1 p.m., especially if poor quality.  We take extra time with difficult audio to ensure we got everything possible.  Again, we sometimes work miracles, so it doesn’t hurt to ask, but the more time you allow us, the more relaxed we can be knowing we are giving you our best and not a rush job where something always suffers.  And sending that stat at midnight for return the next morning, even if only 10 minutes long, really is a difficult task for us.  We occasionally sleep too, but if you give us a heads up early in the day to expect it, guess what – we can do that!

 

Lastly, if you send a file and we do not confirm receipt within 15 minutes, there may have been a glitch in the e-mail.  We always confirm receipt – usually within 15 minutes.  If you don’t hear from us, verify receipt.  And when you send us a file today of less than an hour, if you do not have it by tomorrow – check with us.  We rarely have a file of an hour or less for more than 24 hours.  These things really bother us because we do not like you to think we aren’t doing our best to assist you meet your deadline.
